I have gone from level 10 to 14 with a demon hunter and level 10 to 16 with a witch doctor just trying to get the House of Curios to spawn with no luck (spent at least 6 hours on this; I'm nothing if not stubborn). I've tried restarting from each of the first 3 quest waypoints (town, bridge, den), so I don't think that's the problem. I seriously doubt it's a simple 1/7 chance...
here's how I found it
1. Start the Sword of a Stranger quest and take the waypoint to Cemetery of the Forsaken.
2. Go through the northwest gate into the Fields of Misery and you'll reach a checkpoint.
3. Now refer to the map above, you can plot a zig-zag path through all the random spawn points. Start zigging to the first two areas northwest from the checkpoint, then zag northeast to the one labelled Lonely Dwelling, zig to the next dungeon northwest (probably a waypoint), then go straight north to the area just east of the lumberjacks. You can probably skip the large Forsaken Grounds tile but just in case, run past with it on your right while heading north to the lumber jacks just to make sure it didn't spawn there. Skip the sheltered cottage area it will never spawn there.
4. If you don't find it by the time you reach the last area near the lumberjacks, TP back to town and leave game. Make a new game and you'll start at the checkpoint again. (I find it quicker than waiting 10s to leave the game outside town)
5. Repeat 1-4.
Initially I was just checking the first dungeon area and remaking new games if it wasn't there. At least until I realised I was checking the wrong clump of areas on the map, ie. the small one just left of the checkpoint, which ISN'T a random spot! Then I decided to just zig-zag to the top of the map like I described above and on the first attempt, there it was at the end near the lumberjacks!

I checked in a few days ago (comment #12 above), and I want to reiterate, Ryu's instructions are absolutely correct. I'd done ~25 instances when I posted before, and I've probably done another 35-40 since then, and I finally got House of Curios just now.
To reiterate, Ryu's instructions are correct. Using the map everyone else has linked here, there are ONLY four places HoC can spawn: the three marked "dungeon" and the one marked "Lonely Dwelling." It cannot spawn at either of the patches marked Forlorn Farm or Forsaken Grounds.
From the checkpoint, go NW to the first dungeon marker.
From there, proceed North to the "Lonely Dwelling" marker.
From there, proceed NW to the second dungeon marker.
Finally, proceed North to the third dungeon marker, just east of Bloody Lumberjacks.
If at ANY POINT you encounter a random tile (the trapped scarecrow, the destroyed camp, the 4 empty tiles (a small fenced area, a small unfenced area, a burned tree stump or abandoned camp)), STOP AND RESET. There is no point in progressing further in your search.
Good luck, keep after it, you will find it eventually.

I wanted to post my experience on here, since this was as much of a nightmare for me as it has been for everyone else.
I just finished Act I on nightmare and decided to clean up some achievements. I left my quest progression as it was, right at the end of Act I. I simply warped to the Fields of Misery each time I made an attempt.
Fast forward roughly 30 or so attempts and I was still completely unsuccessful. I finally decided to change my quest to "5. Sword of the Stranger (Begin Quest)." I started in the cemetery and worked my way north to the southernmost "random dungeon" in the Fields of Misery. When I got there, I found the scarecrow. But for kicks, I decided to work my way to the next spot north, just to see what was there. And lo and behold, I found the House of Curios.
This is contradictory to what I've read throughout this thread. I decided to check out the remaining two dungeon spots. In all, my map had the scarecrow, the House of Curios, the Kharza Den, and the Waypoint. I did not find the Scavenger's Den/Lost Mine. Take all of this FWIW...
Anyways, here are two things that appeared to make a difference for me:
1. Change your "active" quest to Act I: Quest 5.
2. Don't stop once you find one of the random landmarks.
Edit: In fact, I might venture to suggest that the House is included in the spawn-pool of Scavenger's Den and Lost mine. Has anyone found a map that had the House AND either of the other two? If this is true, then the strategy might change to quit once you find the Den or the Mine, not any of the other garbage...

+1 here. I have taken screenshots and will make a youtube video of it soon, there was no scavengers den or lost mine.
I had the burnt tree AND the house of curios... as well as the usual Khazra Den and Waypoint, my two large tiles were the pointless hill / water hole, and the Moon Clan Base / Meeting
& (think this is always there?)
Did one map clear in hell and got all this stuff to pop in fields of misery. Perhaps I wasnt understanding what was being said but it seems alot more then 4 tiles spawn.
Decaying Crypt and Forsaken Grounds are big adventure tiles, Khazra Den, Lost mine and burning tree plus the waypoint are your four small adventure tiles.
Got my House of Curios on 20-something tries on Nightmare. Always picked last quest on Act I as starting point (meaning it's not related to which quest you pick, as long as you have access to Fields of Misery). Mine popped on the very first small adventure tile, started from cemetery every time. Had these:
Big tiles: Farm with cellar and Secluded grove.
Small tiles: Khazra den, waypoint and House of Curios, didn't check most northern small tile, sorry.
I don't think any of the information posted in this thread is 100% correct. After at least 75 runs (including 30+ full clears I have yet to see the HoC. This could either mean it's bugged in recent days (and won't spawn at all until fixed) or the actual chance of it spawning is extremely low (and some people just got lucky). Let's not assume this location or any other has a 1 in 7 chance of appearing somewhere in a specific set of tiles. The HoC could very well have a 1% chance of spawning and other locations have a 15-20% chance, for example. We just don't know enough about the underlying game systems or the math involved.

After about 50 shots of trying for this i finally discovered it on a new monk while i was looking for the den. I'd decided to goto the cottage up top of the map hoping for the trader and found House of Curios just slightly south of it.
