What attack counts as melee?

For examples, do these count as melee or ranged? I'd like to know whether melee damage mitigation works on them.
*Fallen Maniac suicide bomb
*Soul lasher/ripper tongue attack
*Herald of Pestilence poison pool attack
*The fire breath from an Oppressor
*Ghom spitting acid on you when you are close range.
*Belial's arm smash (phase 3)
*Belial's exploding green circles
*The DOT when Diablo grabs you
*"Desecrator" circles
*"Plagued" circles
*"Molten" lava path
*The explosion after molten elites die

I guess if thorn (physical damage to melee attacker) activates, it counts as melee.. but usually my hydra is also damaging them so it's very hard to see.

btw do all damaging attacks fall into either melee or ranged?
Thorns has a little animation, it looks like 4 little reddish swords/daggers going into the enemy. It's brief but noticeable. I wish I could help you out but I don't know the answers to any of these, sorry.

Using Ice Armor Crystallize, I have found out whether these are melee attacks:

YES *Fallen Maniac suicide bomb
YES *Soul lasher/ripper tongue attack
NO *Herald of Pestilence poison pool attack
NO *The fire breath from an Oppressor
but each charging hit from oppressor counts as TWO melees.
NO *Ghom spitting acid on you when you are close range.
but YES on Ghom biting you
YES *Belial's arm smash (phase 3)
YES *Belial's exploding green circles
YES *All of Belial's phase 3 attacks are melee.
NO *The DOT when Diablo grabs you
NO *"Desecrator" circles
NO *"Plagued" circles
NO *"Molten" lava path
YES *The explosion after molten elites die
New discoveries:
YES *Siegebreaker charging attack
NO *Siegebreaker grab and throw attack and some stabbing attacks
YES *Explosion traps (like the ones in Siegebreaker and Arreat crater)
YES *Arcane lasers
YES *Fire spewing structures (in Arreat crater)
NO*Fire chains

tested in Nightmare.

It's widely believed that fire chains and mortars are also physical (not fire)... now arcane sentry are also physical?????? Confused.

Arcane sentry triggers ice armor. It is not necessarily physical damage type.
No, they are melee attacks, which doesn't necessarily make them physical. Two different things.

Arcane Sentry's damage is also affected by the affix 'Reduce damage from melee attacks by x%'

arcane sentries are 1) melee 2) arcane 3) elite
hmm, it seems this resource needs to be updated for 2.04 new affixes.

it may seem obvious that these aren't melee, but then you never know.

jailer instant dmg
thunderstorm
poison enchanted
vortex instant dmg
frozen pulse
orbiter

Tested again today (patch 2.0.4). Same method as before (Crystallize) but in normal.

does that mean that these affixes are "range" or is there a 3rd category outside of melee/range that these would fall into?

Also, for "reduces damage from elites" does that include ground effect damage or just the damage from if the elite hit you directly?
