At the end of lord of destruction, the census amongst the townsfolk in horragoth, including Cain, was that the !@#$ would hit the fan with the world stone destroyed.
It would break the barrier between heaven, hell and the mortal realms.
Well that didnt quite happen did it? There was 20 years of quiet.
Anyone find that strange.
20 years does seem like awhile, but in the face of eternity maybe it's not. I can't answer why Hell did not immediately invade, but Heaven might have paused to wait for Tyrael. Demons did infiltrate Sanctuary quite a bit for 20 years, but it wasn't serious business like Arreat.

Sure bring all of them, it will leave a way for them to be defeated.
#2 (The more accurate reason imo) - Time in sanctuary flows like we are aware, but in compared to the high heavens and the burning hells they have a lot longer. Something that might be very quick to them will seem like an eternity for us.
