If the World Stone inhibited the powers of humans, and wasn't destroyed until the end of D2Ex, we're the PCs before nephelem or not? they were definitely powerful, but the lore books, particularly the ones referring to durial, andariel, mephesto, baal, and diablo, in D3 just refers to them as a band of heroes.
Inarius did indeed alter the Worldstone in such a way that "killed off" the Nephalem's power as generations went on, resulting in lowly humanity. This went on for an unknown amount of time, but likely to be at the very least a few hundred years if not upwards of 1000+ until Uldyssian, the leader of the second coming of the Nephalem once again altered the Worldstone every so slightly. Up until then, the only way for humans to unlock their Nephalem birthright was to have it awakened by those who knew how to do it (Lilith, Uldyssian, and Serenthia if I'm not mistaken). However, the alteration Uldyssian made to the Worldstone allowed for a few people every generation to be naturally born with the Nephalem birthright. This includes every PC from every Diablo game, as well as other notably strong humans.
