This form is used to recover or change information on a Battle.net account. Please do not submit this form until asked to do so by a Blizzard Entertainment Customer Support Representative.

Note: Before submitting this form, ensure that you have access to the account’s registered email address. If the registered address has been changed without your authorization, please contact Customer Support via phone for assistance.

Instructions

1. Print this form and complete the entries accurately and clearly in blue or black ink.
2. Attach all requested documents. See the attachment guidelines below.
3. Sign the form by hand. Digital signatures will not be accepted.
4. Submit the scanned form via a Battle.net Support Site ticket.
5. Include the Battle.net account name and details of the service you would like performed in the description field.

ID Submission Instructions

Clearly scan or photograph the registered player's government-issued identification. The identification must be valid. We do not accept expired identification.

Attachment Guidelines

You may attach up to four files to a Battle.net Support Site ticket. Attachments must:

• Be under 5MB
• Be BMP, JPG, GIF, TXT, SPX, or TGA format

Files with other formats or files that are too large may be filtered or deleted without response.

The transfer of a game account between individuals may be allowed under exceptional circumstances, such as a transfer from a parent or guardian to a child upon their reaching the age of majority. If you wish to initiate an account transfer for such a circumstance, this section is to be completed by the current account holder.

Note: Blizzard reserves the right to refuse any account transfer request.
