how do I transfer them from one toon to another on a different server ?

You're only current option is to transfer a character using a paid character transfer with the item in its inventory.

You put the heirlooms on a level 10 character or higher and pay to transfer them. That is the only way to currently do it.

At the moment, that would require they be transported on a character during a transfer.

This is something on the horizon but is NOT an easy task. It's not yet live but hopefully will be implemented, it's something our developers have stated that they'd like to implement.

Maybe it's just a confusing name. Why not name it "server bound", so players know it sticks to the server and not to the account.

Because if you want to move it through a transfer off of the server, you can. All you need to do it keep it in the bags of the toon you are transfering.

Because they are not bound to a specific server. If you transfer to a new server with BoA's in your inventory, they go with you to the new server.

Also, the designation of the items are "Battle.net Account Bound". Each battle.net account can hold up to eight (8) WoW licenses. And thus, these items can be sent between the accounts via the mail system, as long as the characters are on the same realm.
