I think I remember during my broken promises testing that the 2nd strike of WotHF would apply two stacks of striken and the 3rd strike would occur during the ICD. Might be what you are seeing.well maybe that also, but if second hit puts 2 stacks than that means first and third doesn't puts no stack? since in long periods testing i got still WotHF stacking 1.5 times less so kinda only on 2 stacks from its 3 supposed attacksI notice no damage numbers showing for pretty much every skill when I do some testing.
