Blizzard likely has data on what hairstyles, facial styles, etc people actually use in game. Fact is, if you got a large portion of the playerbase using only a couple of the options consistently, what motivation is there truly to add more options? It's like when people assume you'll see a variety of character appearances if there were sliders when in reality, players gravitate to what is "prettiest."
