The gnomish death ray packs a delicious punch even though it has an insane cool down. And the trinkets that let you reflect certain schools of magic make PvP and some PvE very very interesting and tilting for your opponent(s)

I still carry around my Goblin Jumper Cables XL
Cannonball Runner
I have a few in mind, but Iron Grenades are the big one.

If you weren't an engineer in classic you were hardcore gimping yourself in pvp, full stop. If you wanted to be successful then engineering was something you had to do, and the grenades are one of the big reasons for that. They're also not mindless to use either. They take some time to throw and an enemy moving around during your throw could mess you up if you don't predict where they're going to be properly.
Carrot on a Stick + Riding Speed to Gloves + Mithril Spurs. Probably not what you were looking for, but great to have.
Arcane bombs, any spell reflector was hilarious, engineering deflector shield, goblin rocket helm, blacksmithing horseshoe trinket, skull of impending doom OH, exploding sheep ( known to destealth rogues in classic wow), magic dust from westfall, tidal charm ( a must have for shamans but really good on any class), sapper charges, shard of the fallen star from AQ.
Stormpike insignia that let you tele back into your base to screw up the whole enemy raid.
