Access to the Wrath of the Lich King is included in your World of Warcraft subscription. The Wrath of the Lich King Heroic and Epic Upgrades are optional purchases.

How to claim the rewards
Once you purchase the upgrade, talk to any innkeeper in a capital city or to Landro Longshot in Booty Bay.
